Registration Instructions

  3. Attach a recent portrait photo.
  4. Hyperlink any necessary documents, if applicable.

  5. Save the completed form as a PDF file, ensuring the file size does not exceed 1MB.

  6. You may reduce the file size via your app settings or a trusted online PDF compression tool.
  7. Click the "Register" button and fill out all required information.
  8. Upload the accomplished Membership Requirement Sheet at the end of the form.
  9. Submit the registration form to complete the process.
